- Aged 18 to 79
- Recently discharged home after a heart-related event (like heart attack or stenting)
- LDL (bad) cholesterol at a certain level depending on whether you take a statin and how strong it is: ≥130 (no statin), ≥110 (moderate statin), or ≥100 (high statin)
- No prior treatment with PCSK9 inhibitors (evolocumab or alirocumab) or inclisiran
- No severe kidney disease (eGFR <30) or active liver problems
- Willing and able to use a smartphone or tablet for the study app (if in the intervention group)
