- You are 18 years old or older.
- You have a type of heart disease called atherosclerotic cardiovascular disease (ASCVD), such as a heart attack, chest pain from blocked arteries, or heart failure caused by reduced blood flow.
- You have finished phase II of cardiac rehabilitation and are now ready for phase III (the maintenance phase).
- You can use digital devices like a smartphone, tablet, or computer, or you have someone to help you with them.
- You are not pregnant, nursing, or planning to become pregnant during the study.
