Who can take part
- You were diagnosed with prostate cancer that hadn't spread outside the prostate between January 2018 and December 2023.
- You were 18 or older at diagnosis.
- You completed one of these treatments at an eligible hospital: radiation therapy (with or without hormones), prostate removal surgery, or focal therapy.
- You were alive and cancer-free 6 months after finishing treatment.
