Who can take part

  • You have been diagnosed with type 2 diabetes.
  • You have at least one of these: coronary artery disease, peripheral artery disease, heart failure, diabetic kidney disease, diabetic eye disease, a past stroke, or atrial fibrillation.
  • If you are 55 or older, you need two of these: you smoke, you take blood pressure medicine or have high blood pressure, you take cholesterol medicine or have high cholesterol, or you have a BMI over 25.
  • Your kidney function must be at least a GFR of 30, and your urine protein level must be 200 or less.
  • Your BMI must be 50 or less.
  • You have a heart ejection fraction of 30% or more, no active cancer, can see, and are able to use digital apps.
