  • You are scheduled for planned (not emergency) heart surgery
  • You have heart failure with a high NTproBNP level (a marker of heart strain)
  • You are 18 or older and agree to participate
  • You can communicate well and have help at home if needed to share health data
  • You do not have severe mental health or addiction issues that would make it hard to follow the program
