This trial tests a program to help stroke survivors better manage their own health after leaving the hospital. If you've had a stroke and are stable, you and a family member would attend educational sessions to learn skills to support your recovery.

  • You are 18 or older
  • You had a stroke (first or repeat) that was either ischemic or from bleeding in the brain
  • Your doctor says you are stable after the initial stroke recovery phase
  • You can speak and read Vietnamese
  • You have a family member or caregiver who can join you and help notice changes in your health
