  • You are between 60 and 75 years old
  • You have been diagnosed with high blood pressure by a community doctor
  • You can read and understand basic information and communicate clearly
  • You are able to use a smartphone
  • You are not yet fully active in changing your lifestyle but are thinking about it or getting ready to start
  • You have not had serious heart, brain, or kidney disease, or cancer, and do not have a mental health condition
