Who can take part
- You are older than 50 and have at least 2 risk factors, or older than 60 with at least 1 risk factor
- You have high blood pressure, diabetes, kidney disease, or other listed heart risks
- You are not currently on dialysis or living in a nursing home
- You do not have active cancer treated within the last year
- You have not had an organ transplant and do not have dementia or amyloidosis
