- At least 18 years old
- Had prostate cancer surgery more than 6 months ago
- Cancer has come back (shown by a PET scan called PSMA PET)
- Cancer has not spread to distant parts of the body
- In good enough health to tolerate radiation (ECOG score 0–1)
- Have not had previous pelvic radiation therapy
