Who can take part
- Age 18–75 years old
- Diagnosed with advanced rectal cancer (stage T3–4 or with lymph node involvement) with tumor within 10 cm of the anal opening
- No prior cancer treatment (surgery, chemotherapy, radiation, or immunotherapy)
- Planned to have surgery after treatment and medically fit for surgery
- Good overall health (ECOG performance status 0–1) with normal blood counts and liver/kidney function
- Not pregnant, not taking immunosuppressive medications, and no active infections or autoimmune diseases
