- Age 18–75 years old
- Rectal cancer diagnosed by biopsy, located in the middle or lower part of the rectum, within 10 cm of the anal opening
- Cancer has not spread to distant organs; it is locally advanced with specific high-risk features seen on MRI
- No previous chemotherapy, radiation, or immunotherapy for any cancer
- Good overall health (able to perform light daily activities) and expected to live at least 2 more years
- Normal blood counts, liver, and kidney function; able to take oral medications
