  • Age 18–75 years
  • Rectal cancer confirmed by biopsy, located low in the rectum (within 10 cm of the anus), with specific genetic markers (pMMR, MSI-L, or MSS)
  • High-risk features on MRI scan (large tumor, extensive lymph node involvement, blood vessel invasion, or fascia involvement)
  • Good overall health and ability to tolerate radiation, chemotherapy, and immunotherapy
  • No prior cancer treatment (surgery, radiation, chemotherapy, or targeted therapy)
  • Normal blood counts, liver, and kidney function; no active infections; no untreated hepatitis B or C
