Who can take part
- You are between 40 and 69 years old
- You were diagnosed with type 2 diabetes (not type 1 or gestational) between January 2022 and December 2023
- You started diabetes medication within a few months of your diagnosis
- You have not had a heart attack, stroke, or blood vessel disease before your diabetes diagnosis
- You are not currently pregnant
- You are willing to have a follow-up visit about 12 months after your diagnosis
