  • You had heart surgery (bypass or angioplasty) and were recently discharged home without needing a breathing machine or intensive monitoring
  • You are medically stable and recovering well without serious complications like severe infections or heart rhythm problems
  • You have a family member or close friend who will be your main caregiver and can participate in the study with you
  • Both you and your caregiver can understand instructions, follow a care plan, and answer questions about your recovery
  • You and your caregiver are willing to sign a consent form and commit to the nursing intervention program
  • You are not currently in another medical study or recovery program that might conflict with this one
