  • You had heart surgery (bypass, valve repair, or aortic surgery) within the last 12 months
  • You are no longer having regular visits with your heart surgery team
  • You are able to understand and agree to participate
  • You are not currently a prisoner
  • You are either the patient who had surgery or a family member/caregiver of that patient
