This study tests whether modifying your gut bacteria can help immunotherapy work again in advanced kidney cancer that has stopped responding to initial immune-checkpoint inhibitor treatment. Researchers will give you a special bacterial preparation along with a new immunotherapy drug to see if this combination can slow or shrink your cancer.

Who can take part
- Biopsy-confirmed metastatic (spread) kidney cancer, clear cell type or certain non-clear cell types (papillary, chromophobe, unclassified)
- Cancer that worsened during or within 6 months after at least 2 cycles of PD-1/PD-L1 immunotherapy
- No more than 2 previous cancer treatment lines total, with only 1 being PD-1/PD-L1 immunotherapy (which must be your most recent treatment)
- At least one measurable tumor lesion on imaging that can be accurately tracked
- Recent tumor biopsy with at least 75% viable (living) cancer tissue
- Good overall health: age 18+, able to care for yourself with minimal assistance, expected to live at least 6 months
