  • You have been diagnosed with type 2 diabetes.
  • Your most recent blood sugar test (A1c) was above 8% in the last 6 months.
  • You have seen a doctor at West Virginia University clinics in the past 6 months.
  • You have trouble affording enough food (screened for food insecurity).
  • You are able and willing to sign a consent form to join the study.
