Who can take part

  • You have been diagnosed with high blood pressure (hypertension) in the last 5 years.
  • You are a patient at UT Physicians Victory location and have seen your primary care doctor there in the past year.
  • You can join virtual visits using a smartphone, tablet, or computer with a stable internet connection.
  • You are willing to check your blood pressure at home on a regular basis.
  • You do not have unstable heart disease, advanced kidney disease, active cancer, severe memory problems, uncontrolled mental health conditions, or very hard-to-control blood pressure.
