Comparing two test-guided treatments for acid reflux

This trial compares two ways to choose the best treatment for people with frequent acid reflux. It tests whether using a simple test (called PCAB-guided) is as good as a more detailed test (called physiologic test-guided) for deciding on the right medicine. The goal is to find a more practical and effective approach.

Who can take part

  • You are between 18 and 70 years old.
  • You have heartburn, regurgitation, or chest pain from acid reflux at least 4 times a week for more than 4 weeks.
  • You do not have complications from GERD like a narrowed esophagus or Barrett's esophagus (a change in the lining of the food pipe).
  • You have not taken certain acid-reducing drugs (called PPIs, H2RAs, or PCABs) in the last 2 weeks (or are willing to stop them for 2 weeks before the trial starts).
  • You have not taken aspirin, NSAIDs (like ibuprofen), stomach protectants, or other specific medications (like prokinetics or neuromodulators) in the last 4 weeks.
  • You are not pregnant or breastfeeding, and you have no known allergies to the study drugs (vonoprazan, omeprazole, or nortriptyline).
