Neratinib plus hormone therapy for advanced breast cancer with low MLH1
Part of Cancer, Skin clinical trials.
This trial tests adding a drug called neratinib to standard hormone therapy for people with a specific type of advanced breast cancer (ER+/HER2-, low MLH1). The goal is to see if this combination works better than hormone therapy alone to slow or stop the cancer from growing.
Summary written for real people, not researchers, by Clin2.
Who can take part
- You are a woman aged 18 or older
- You have metastatic breast cancer that is estrogen-receptor positive (ER+) and HER2-negative
- Your cancer has gotten worse after at least one line of hormone therapy (with or without a CDK4/6 inhibitor)
- A recent biopsy (within 6 months) shows your tumor has low levels of the MLH1 protein
- You are not pregnant, planning pregnancy, or breastfeeding
- You have at least one tumor that can be seen on a scan, can be biopsied, and has not been treated with radiation or other local therapy

This trial tests a combination of two targeted drugs, trastuzumab deruxtecan (Enhertu) and neratinib, for people with solid tumors that have a HER2 gene change. The goal is to see if the combination is safe and works well for cancers that have not responded to at least one prior treatment.
This trial tests whether taking neratinib after completing a year of standard treatment can help prevent breast cancer from coming back in women who had a complete response to initial therapy but still have high-risk features.
This study tests whether a pre-surgery treatment plan that adds neratinib to hormone therapy and trastuzumab can shrink HER2-positive, ER-positive breast cancer before surgery. It may help patients by improving how much cancer is eliminated at the time of surgery.
This trial studies whether adding neratinib to standard trastuzumab treatment after neoadjuvant (before-surgery) therapy lowers the risk of breast cancer coming back. It is for people whose breast cancer is hormone-receptor positive and HER2 positive, with residual cancer found after neoadjuvant treatment.
This study tests a daily pill called neratinib for people with advanced cancers that have a change in the HER2 gene. It may help treat tumors that are no longer responding to other treatments.
This Phase 2 trial tests neratinib given before surgery to treat stage I–III invasive lobular breast cancer that has a specific HER2 mutation and is hormone-receptor positive. It may help shrink the tumor and improve control of cancer before and after surgery.
