  • You have had a brain blood-vessel condition that fits a stroke-related definition (including problems from blocked blood flow or bleeding).
  • Your diagnosis falls within certain ICD-9 categories (ICD-9 430–438), which your doctor can confirm.
  • You (or your legal representative/parent or guardian, if needed) can provide informed consent to join.
  • You are not planning to leave the study or withdraw your consent.
  • Your stroke or cerebrovascular disease started on or after January 1, 2016 (not earlier).
