  • You are age 18 or older.
  • You had cardiovascular (heart/blood vessel) surgery at UC Davis Medical Center.
  • Your surgery date was between January 1, 2001 and July 15, 2013.
  • Your surgery involved heart surgery, with or without a heart-lung machine (cardiopulmonary bypass).
