Who can take part

  • You have severe aplastic anemia (bone marrow under 30% cellularity) with low white blood cell counts and/or needing red blood cell or platelet transfusions, OR you had severe aplastic anemia that turned into MDS with specific risk and marrow/blast counts.
  • You have not responded well to immune-suppressing therapy (like ATG and cyclosporine, and/or cyclosporine with eltrombopag if you couldn’t get ATG or couldn’t tolerate it).
  • You must have a backup donor option available, such as a compatible haploidentical or matched unrelated donor, or extra cord blood units if the main cord unit isn’t usable.
  • You need access to an eligible cord blood unit from a national bank with good matching and enough cell quantity (there are special rules if the total cell amount is lower but CD34 cell counts are high).
  • You must be between ages 4 and 60 and be able to understand the study (parents/guardians help with consent for ages 4–17).
