- You are a man age 18 or older
- You were diagnosed with cancer or a blood (hematologic) disorder before age 18

- At least one year has passed since your last high-risk (genotoxic) treatment
- When you were younger, you either did or did not have a testicular tissue biopsy to try to preserve fertility
