Who can take part
- You’re age 21 or older and willing to sign consent for the study.
- You have essential tremor (a shaking condition) for at least 3 years, and your tremor affects an arm/hand you want to treat.
- You already have the usual VIM DBS setup, and your tremor is still clearly disabling after at least 3 months on it.
- Your tremor scores must meet minimum levels, including at least 2 out of 4 severity in the target arm/hand and difficulty with at least one daily activity (like writing, dressing, or feeding).
- You’ve tried at least two tremor medicines (including propranolol or primidone) at effective doses or had side effects from titrating them, unless you already have a device being replaced.
- You can come to follow-up visits during the study and follow study directions.
- You have no other brain problems on MRI (no stroke, major structural issues, hydrocephalus, infection, severe brain shrinkage).
