Who can take part
- You are a man age 21 or older and can sign consent and follow study instructions
- Your prostate cancer is higher-risk (or unfavorable intermediate risk) based on tests like Gleason score, PSA level, and/or tumor stage
- Your scans show no spread (no metastatic disease), including no pelvic lymph node spread
- You must be able to receive the PSMA IV medicine used for the PSMA PET scan
- You cannot have certain prostate or treatment issues that make SBRT unsafe, such as prior prostate radiation, very large prostate, or very high urinary symptom score
- You cannot be taking abiraterone, and if you are sexually active you must use birth control and condoms for 7 days after each PSMA injection
