- You have either heavy protein in your urine or advanced kidney disease
- You currently have fluid overload (swelling in legs, belly fluid, or fluid around the lungs)
- Your “dry weight” is at least 5 pounds less than your current enrollment weight
- You can measure and record home blood pressure and body weight, and you have a working smartphone
- You are not pregnant and you are between 100 and 300 pounds
