Who can take part
- Be 18 years or older and either admitted to the hospital or seen in the emergency department
- You must have been diagnosed with heart failure by the study’s methods (including Framingham criteria and/or a BNP blood test)
- Your BNP blood test must be above 100 (100–400 counts as “possible” and above 400 counts as “definite” heart failure)
- You can (or someone can) provide informed consent to join the study
- You must live in Ontario (not another province/country)
- You must plan to go home after discharge (not to a skilled nursing facility/long-term care)
