This study tests a new outpatient care approach for people with heart failure to help manage symptoms like shortness of breath and fluid buildup. You may be eligible if you are stable enough to attend visits and meet specific heart failure and lab/imaging criteria.

Who can take part
- You have shortness of breath (with activity or even at rest) from heart failure
- You also have at least two signs of fluid overload (for example, on chest X-ray or exam, or with swelling)
- Your blood test supports heart failure (NT-proBNP at least 300, or at least 900 if you have atrial fibrillation)
- You have been an outpatient (not requiring continuous inpatient care)
- You do not have severe untreated heart valve problems or require intensive shock treatment (inotropes)
- Your kidneys are not in end-stage failure (eGFR must be 15 or higher, and you should not be on dialysis)
