Who can take part

  • You have shortness of breath (with activity or even at rest) from heart failure
  • You also have at least two signs of fluid overload (for example, on chest X-ray or exam, or with swelling)
  • Your blood test supports heart failure (NT-proBNP at least 300, or at least 900 if you have atrial fibrillation)
  • You have been an outpatient (not requiring continuous inpatient care)
  • You do not have severe untreated heart valve problems or require intensive shock treatment (inotropes)
  • Your kidneys are not in end-stage failure (eGFR must be 15 or higher, and you should not be on dialysis)
