- You are 18 years old or older.
- You have finished treatment meant to cure your prostate cancer.
- You get prostate cancer survivorship follow-up care at one of the listed partner hospitals/clinics.
- Your cancer specialist says you are low risk for recurrence (cancer coming back).
- You can do study activities in English, or a caregiver can help translate.
- You have (or can use) an email address for study contact.
