- You can sign written consent and follow study instructions
- You have an MPN diagnosis that is BCR-ABL negative
- Your MPN has progressed to an accelerated phase or blast phase, and you have not had prior blast-reducing therapy for that stage
- Your general health is good enough for study treatment (ECOG 0–2)
- Your organs (kidneys/liver/heart) can handle study medicines (adequate organ function)
- If you can become pregnant: you must use reliable birth control and have a negative pregnancy test before starting
