  • You are 70 years old or older
  • Your blood pressure has been high (at least one reading with top number ≥130 or bottom number ≥80) in the past 24 months
  • You saw your primary care clinic doctor within the last 24 months
  • You can read and speak English or Spanish
  • You own a smartphone and are willing to participate and consent
  • You do not have dementia or major neurological disease
