  • You had a cardiac arrest outside the hospital that was treated successfully and you were admitted to the ICU
  • You are 18 years of age or older
  • Your cardiac arrest was not caused by a traumatic injury
  • Your cardiac arrest was not caused by drowning
  • Your cardiac arrest was not caused by electrocution
