Who can take part
- Your baby was born before 32 weeks of pregnancy
- Your baby has lung disease that needs breathing support for 28 days after birth (at least mild BPD)
- Your baby is being discharged to their home (not to another facility)
- Your baby may be on oxygen to go home (that can still qualify)
- Your baby does not have a major other congenital condition (heart, genetic, or nerve system) that is thought to be a main cause of the breathing problems
