- You are over 18 and this was your first STEMI (major heart attack).
- You had a successful emergency heart procedure called primary PCI, and you can consent.
- Your heart pumping is reduced (left ventricular ejection fraction under 50%) on imaging after the PCI.
- Your heart attack area is large enough (infarct size over 10% of the left-ventricle mass).
- Imaging shows microvascular blockage (MVO) over 10% of the infarct size.
- You must be able to safely have a cardiac MRI (no MRI contraindications or gadolinium contraindications).
