Who can take part
- You’re age 18 or older (or able to consent by local law).
- Your head and neck cancer is in the mouth, throat, voice box, or is an unknown primary that tested p16-positive, and it’s locally advanced.
- Your cancer meets specific HPV risk group and stage rules (HPV negative stage III–IVB, or HPV positive stage III).
- Your tumor test must show PD-L1 (CPS 1 or higher), using a specific lab test.
- You must be able to get definitive treatment (surgery plus radiation/chemoradiation, or radiation/chemoradiation).
- You must be well enough for treatment (performance status 0–1), have a certain blood/organ function, and have enough time to live at least 12 weeks.

This Phase 2 trial studies whether 5-azacytidine and nivolumab (alone and/or together) can work well and be tolerated in people with HPV-related throat cancer that can be surgically removed. The goal is to help shrink or control the cancer before resection and learn more from added tumor samples.
This trial is for people with head and neck cancer that has not spread to distant parts of the body. It is a non-treatment study that will look at the molecular features of your tumor from a biopsy sample to help guide future treatment decisions.
