Who can take part
- You are over 18 years old
- You have type 2 diabetes, either diagnosed already or newly diagnosed
- You are using diabetes medicines by mouth and/or injections (such as insulin or GLP-1 medicines)
- You were admitted to the hospital cardiology unit with an ACS (acute coronary syndrome) event, and your blood test troponin was high
- Your HbA1c (an average blood sugar test) is at least 48 mmol/mol
