Who can take part
- You are age 60 or older
- Your recent heart ultrasound shows preserved pumping strength (ejection fraction 45% or higher)
- You were in the hospital more than 24 hours for worsening heart failure, or you were diagnosed with heart failure after another hospital stay
- Before this hospital admission, you could do basic daily activities and walk on your own (with or without a walker)
- At enrollment, you can walk 4 meters (about 13 feet), with or without a walker
