Who can take part

  • You are 65 years or older.
  • You were recently in the hospital for heart failure and are now stable (off strong heart medications for at least 3 days).
  • You can walk more than 4 meters (about 13 feet) on your own.
  • Your heart failure symptoms are class II or III (moderate symptoms like shortness of breath with activity, but not at rest).
  • You do not have advanced dementia, a life-threatening illness, or a condition that prevents you from exercising safely.
