  • You’re an adult (18+), and you have diabetes type 1 or type 2
  • You live in the area served by local Family Health Units (primary care clinic area)
  • You can join the program at the scheduled group times
  • You do NOT have a diagnosis of dementia
  • You are able to answer questions clearly and reliably (you can follow instructions consistently)
