Who can take part
- You are a patient of Northwestern Medicine
- You are 18 years or older
- You meet at least one cancer risk factor: low activity, higher BMI, or current/recent smoking (within the last year)
- You can join telehealth visits by phone or video (cell phone, smartphone/tablet, or computer/laptop/desktop)
- You are not already in another weight loss, diet, smoking quit program, or physical activity program
- You can safely participate: no major unstable heart/lung/medical restrictions, and you are not pregnant or planning pregnancy
