Follow-up study for the NuVasive Simplify Disc implant

  • You were enrolled in the Simplify Disc earlier study at a participating clinic (either IDE NCT03123549 and/or PAS NCT04980378).
  • You understand what this follow-up study involves and are willing to sign a consent form.
  • You did not have an SSI at the implant site during your earlier IDE or PAS study.
  • You have not been withdrawn from the earlier study, and you did not withdraw your consent.
