Who can take part
- You are 18 or older and recently left the hospital/urgent care after a heart failure visit (or were referred to cardiology right after).
- Your heart pump test showed ejection fraction under 50% within the last 3 months.
- Your blood test NT-proBNP was above 900 during the hospital stay or shortly after discharge from the ED.
- You (or a caregiver) have a mailing address for study materials.
- You can give verbal consent and participate in follow-up, including using digital health tools.
