  • You are 18 or older and have been recently in the hospital.
  • You have been diagnosed with heart failure (new or existing).
  • Your doctor knows your ejection fraction and NYHA class (a measure of your heart function and symptoms).
  • You or your carer can speak and understand English or Irish.
  • You live in the Saolta group area (west of Ireland).
