Who can take part
- You are 18 years old or older.
- You have been newly diagnosed with heart failure (not a previous diagnosis).
- At the time you leave the hospital, your heart failure symptoms are moderate (NYHA class II or III).
- You do not have a history of heart failure before this diagnosis.
- You do not have other serious health problems (like severe lung disease, dementia, or memory issues) that would make it hard to follow the study.
- You are not currently in another clinical trial that tests an experimental treatment.
