Who can take part
- You are 18 years of age or older.
- You have been diagnosed with Alzheimer's disease or a related dementia, mild cognitive impairment, Parkinson's disease, or multiple sclerosis.
- You have had a visit with a healthcare provider at a participating site within the past three years.
- You are 60 years or older at the time of enrollment.
- You (or a legally authorized representative) are able to provide written informed consent.
- If you have a care partner, they may also be invited to participate in surveys and will provide their own consent.
