Who can take part
- You must be an adult with a diagnosis of ADRD, MCI, Parkinson’s disease, or multiple sclerosis
- You must be able to enroll by providing written consent (or have a legally authorized representative)
- You must have had a medical record (EHR) encounter within the last 3 years
- If you agree to surveys (PROs), you generally need to complete them within 24 weeks after your first invitation
- You should not have plans to withdraw consent for surveys during the study
