  • You were referred to the study clinic for surgery for lumbar spinal stenosis or cervical radiculopathy
  • You are 18 years old or older
  • You had an MRI (magnetic resonance imaging) within the last 12 months that matches your symptoms
  • Your doctor and you agree you have signed informed consent
  • Your symptoms are not caused by another medical condition (no other confirmed reason)
