- You must be enrolled in the Choctaw Nation of Oklahoma (CNO)
- You must live in the CNO tribal region for the past 12 months
- You must say “yes” to at least one habit area, like frequent sweet drinks/fast food, low outdoor time, low active time, or high screen time
- You must not have used opioids or methamphetamine 2 or more times in the past month
- You must be able to walk and take part in exercise (no disability that blocks it)
- You must be able to follow directions and have no current severe psychotic symptoms
