Who can take part
- Be age 40 or older
- Be able to give informed consent (and understand the study)
- Have a previous heart-risk condition such as coronary artery disease, diabetes, or a prior stroke
- Have at least two listed heart-failure risk factors (for example, atrial fibrillation, COPD, chronic kidney disease, or regular water tablets)
- Not already have a diagnosis of heart failure
- Not be on kidney replacement treatment (dialysis) and not be considered unsafe for the study
