- You have high blood pressure (hypertension) that's not well controlled.
- You've been taking blood pressure medication for more than 3 months.
- Your blood pressure is above 130/80.
- You have a mobile phone and can read text messages (with or without help).

- You are not pregnant or just after childbirth, and you don't have serious mental or thinking problems.
